Hospice Services
Hospice care includes all care related to the palliation or management of the terminal illness, as well as other symptoms and conditions as they relate to the terminal disease.
Direct services include:
Nursing care: Frequency and skills determined by the patient/family and Hospice team, available 24 hours, 7 days a week, the on-call system.
Social services: Social worker makes the initial visit with Primary nurse and offers social services to patient/family Frequency of visits are determined by patient/family and Hospice team.
Physician Services: Medical Director attends all team meetings and supervises all care provided by team members, available on call 24 hours a day, seven days a week for orders iff attending cannot be reached.
Counseling Services
Bereavement - Counseling support for up to one year following death of loved one
Spiritual - Pastoral care available regardless of religious affiliation
Dietary - Diet and nutritional counseling available if appropriate
Other as needed - Any other type of counseling deemed appropriate by Hospice team
Home Health Aide and Homemaker Services
Medical Supplies: Including DME as well as other self help and personal comfort items, related to the palliation or management of the terminal illness.
Drugs and Biologicals: Used primarily for the relief of pain and symptom control related to the person's terminal illness.
Physical or Occupational Therapy and Speech and language Pathology: Services are provided for symptom control, maintenance of basic funtional skills, and to provide instruction to the family or staff.
Short term inpatient care at Cooper County Memorial Hospital: In Boonville as necessary for pain control, acute or chronic symptom management and to provide respite for the patient's family or care giver.
Continuous Home Care: Provided during brief periods of crisis to maintain the patient at home.
