PURPOSE: To make available to the hospital patient and their families all services necessary to facilitate their adjustment to their illness and to promote maximum benefits from the health care provided by the hospital and other community resources.

SERVICES RENDERED: Discharge planning, information and assistance in arranging care after hospitalization either in the hospital's extended care, area nursing or boarding homes, or in the patient's own home. Referral for counseling, financial help, services for the terminally ill, and assistance to transient patients.

PERSONS SERVED: Patients in Cooper County Memorial Hospital and their families.

AREA SERVED: Cooper and surrounding counties or any hospital patient

FEES: None to the patient.

HOURS: 8:30 A.M. - 4 P.M., Monday - Friday
