PURPOSE: Help older adults live independently in their own homes with maximum safety and support.

SERVICES RENDERED: Care coordination can involve informal information and referral,

or may need a formal in home assessment. The care coordinator will help elderly obtain needed services

and provide information and assistance in obtaining help from other agencies. Services the care

coordinator may help with include: home delivery meals, respite care, in home care, transportation, l

egal services, companionship, telephone reassurance, sign up for eligible benefits, assistance with

Medicare and Medicaid insurances, taxes, grants, food stamps, SSI assistance.

PERSONS SERVED: Any person age 60 or over in the Central Missouri Area. There are no income or asset restrictions.

AREA SERVED: Audrain, Boone, Callaway, Cooper, Camden, Cole, Crawford, Dent,

Gasconade, Howard, Laclede, Miller, Moniteau, Morgan, Osage, Maries, Phelps,

Pulaski, and Washington counties.

FEES: None.

HOURS: Monday - Friday (Serves Cooper & Howard Counties)
